A distinguishing element to services of CHF is the collaborative structure, built around relationships of community health organizations.  The 16 member clinics provide input through agency representatives and work together to provide patients with resources and referrals to meet individual patient needs.  The wide range of services provided by the group ensures that the majority of patient needs can be met through the collaboration.

Annex Teen Clinic

Annex Teen Clinic

Partnering with young people through age 25 to improve adolescent sexual health, and prevent unintended teen pregnancy and sexually transmitted infections. Our goal is to foster authentic youth-adult partnerships in the service of our mission.

Mission:

The mission of the Annex Teen Clinic is to support the young people of Northwest Hennepin County and North Minneapolis in the challenging transition from adolescence to adulthood through advocacy, health care, counseling and education.

• A variety of contraceptive methods, including: condoms, hormonal methods, emergency contraception and long acting reversible methods such as the Implanon rod and the IUD (Intra Uterine Device)

CUHCC has free and confidential advocacy services for victims of domestic violence and sexual assault.
CUHCC works with the law firm Leonard, Street and Deinard to help patients with legal matters.
CUHCC has staff who can speak with you in Cambodian, English, Hmong, Lao, Somali, Spanish and Vietnamese. Other languages are available by request, such as Oromo, Amharic, Thai and French.
CUHCC provides culturally competent care by dedicated staff. When we discuss health care decisions, we consider the beliefs and customs of our patients.
